unable to vote at your polling place on election day for one of following
reasons, you may be eligible to vote absentee.
If you are eligible to vote absentee -
please complete the adobe acrobat Absentee Form and mail to:

* Please complete the yellow
highlighted areas, and mark the appropriate box (that explains why you are
unable to vote at your polling place).
Reasons for Voting Absentee
1. You expect to be absent from
your City/County of Residence (including students temporarily abiding
outside the precinct in which he or she is registered to vote due to the
fact that he or she is a student attending an Institution of higher
learning).
2. You are physically unable to
go to the polling place.
3. You are observing a
religious holiday.
4. You are performing certain
election duties.
5. You are serving as a
sequestered juror on a state or federal jury.
6. You are a pre-trial detainee.
